Jay Mayfield Obituary

Jay D. Mayfield, 66, passed away Wednesday, May 10, 2023, at Clark Memorial Hospital. He was born on July 26, 1956, in Floyd County, Indiana to the late Everette and Bonnie J. Mayfield.

He is survived by his children, Jason Mayfield (Melissa) and Tiffany Mayfield (Ben); granddaughter, Katlyn, grandson, Dustin and his great grand-grandchildren, Lincoln and Walker.

Cremation services have been entrusted to Chapman Funeral Home, 431 West Harrison Avenue, Clarksville, IN 47129
